Community Health Improvement Plan

We are very excited to share the Greater Milford Community Health Improvement Plan (revised September 2020) with all of you!
The steering committee of the CHNA was charged with revisiting the CHIP each year to “improve and further align expectations with outcomes.” During the fall of 2020, the steering committee reviewed the CHIP with an eye towards how it could align with the needs that have emerged from the coronavirus pandemic.
What follows is a reimagined CHIP that continues to focus on the goals of the four priority areas identified through the CHA but that identifies ways to address health issues resulting from COVID-19.

2021 Community Health Assessment

Beginning in October 2020, Milford Regional Medical Center (MRMC) initiated a comprehensive
Community Health Needs Assessment (CHNA) to evaluate the health needs of individuals living in its
eighteen-town service area in Massachusetts. The aim of the assessment is to reinforce Milford Regional Medical Center’s commitment to the health of residents and align its health prevention efforts with the community’s greatest needs. The assessments examined a variety of health indicators, focusing on
various health issues affecting the community’s different population groups. Milford Regional Medical
Center contracted with Holleran Consulting, a research firm based in Wrightsville, Pennsylvania, to
execute the CHNA. For the 2021 Final CHNA report, click here

MRMC previously conducted a comprehensive CHNA in 2018 and a Community Health Assessment in 2015 to evaluate the health needs of individuals living in the service area. The purpose of the assessment was to gather information about local health needs and health behaviors. The assessment helped MRMC to identify health issues and develop community health implementation plans to improve the health of the surrounding community. The prioritized health issues that were originally identified in the 2015 CHNA were the continued focused in 2018.
