Community Health Improvement Plan

We are very excited to share the Greater Milford Community Health Improvement Plan (revised October 2018) with all of you! 
 
The steering committee of the CHNA was charged with revisiting the CHIP each year to “improve and further align expectations with outcomes.”  During the fall of 2018, the steering committee worked to evaluate the outcomes of current grantees and how well they met the objectives of the CHIP.  In addition, the steering committee reviewed the 2018 Community Health Assessment conducted by Milford Regional Medical Center. 

 

2018 Community Health Assessment 

Beginning in October 2017, Milford Regional Medical Center (MRMC) undertook a major review and revision of the 2015 CHA to evaluate the health needs of individuals living in its twenty-town service area in Massachusetts. The assessment examined a variety of health indicators, focusing on mental health/substance abuse, senior support issues, access to care, and overweight/obesity. The resulting document is the the 2018 Community Health Needs Assessment (2018 CHNA).

 

The MRMC 2015 Community Health Assessment (2015 CHA) synthesized secondary data, a community survey with over 1,000 responses, and interviews with leaders representing health, government, education, public safety, and faith-based institutions.

The Greater Milford Community Health Network (CHNA 6) is one of 27 Community Health Network Areas (CHNAs) across Massachusetts.  We serve the 13 communities of: Bellingham, Blackstone, Douglas, Franklin, Hopedale, Medway, Mendon, Milford, Millville, Northbridge, Sutton, Upton and Uxbridge.  CHNA 6 is a partnership with the Massachusetts Department of Public Health, the Milford Regional Medical Center, local service agencies, schools, businesses, Boards of Health, community health centers and other health organizations, as well as other concerned citizens who are working together to build healthier communities.
